Foot and Ankle Wellness Center

New Patient Visits:

Please arrive at your appointment 10-15 minutes early, in order to complete the registration process. Please bring the following:

A list of current medications and any known allergies.
Driver’s License or State ID Insurance Information and Insurance Cards.
Copies of surgical records, medical records, X-rays, MRIs, and CT scans when applicable.

Same-Day Appointments:

We offer same-day appointments to both established and new patients. If you need to be seen, we will do our best to see you that day!

Billing Information

Insurance and Payment information:

Co-pays and balances are requested for all office services at the time of the visit. We accept cash, debit cards, checks, VISA, MasterCard, Discover
Most Insurances accepted. It is necessary for you to check with your HMO and/or PPO for any special referrals
Please bring all insurance cards to your appointment, with all necessary and correct information to file insurance claims for you.

Medicare

We are participating physicians with Medicare and accept assignment. We will bill your secondary insurance when needed

Cancellations:

Please give us a 24 hour advanced notification if you are not able to make your appointment.

First time visit today to Dr Habuda. Very caring, thorough Dr. While the injection 💉 in my foot hurt like h--l, he was very compassionate, asking if I was ok, which I appreciated ! His assistant is great also, really nice lady.

I have reservations about writing this review because I’m selfishly afraid he will get so busy it will take weeks to get in. All joking aside, I have never met a Dr. more caring! He is truly the best! I even brought my daughter to him and he was extremely patient with her. Well worth the 30 minute drive for me.
